Report: Patient-safety culture improving in NHS hospitals

A report from the British National Patient Safety Agency showed that more than 3,600 people died from patient-safety incidents, including medication errors, hospital-bug-related events and mistakes in clinical assessments, between 2007 and 2008. Health experts say the increase is because more hospital staff are reporting such incidents.
