The Chronic Care Model, which gives patients tracking systems and self-management skills, showed efficacy in diabetes care and helped boost patient data management and provider responses to clinical data, researchers reported in the CDC's Preventing Chronic Disease journal. Further evaluation may enable health care personnel and administrators to decide how to better coordinate the model with primary care initiatives for diabetes patients.
