Calif. program to prevent readmissions using health coaches

A $4 million CMS-funded program to reduce readmissions among high-risk elderly patients in Ventura County, Calif., will include hiring up to 17 health coaches to work with seniors on medication compliance, self-care and getting to physician appointments. A pilot program run by the Camarillo Health Care District found that using coaches allowed 72% of participating seniors to avoid hospital readmission within 30 days.
