Combination of face to face, online best for exit interviews

Although exit interviews are typically heavy on anecdotes and light on data, blogger Suzanne Lucas recommends getting as many reasons as possible for an employee's departure through a face-to-face interview and online survey. She also recommends asking former employees to fill out a second exit survey six months after leaving the company in order to gain a fresh perspective.
