Health center, NYC HIE coordinate care through alert system

A real-time alert system was developed by Maimonides Medical Center and the Brooklyn Health Information Exchange to send texts and secure online messages to psychiatrists to let them know when their patients are discharged or enter another facility. This could include admission to or discharge from an inpatient facility or emergency room, or an admission for psychiatric reasons. The medical center also plans to extend the use of the platform to aid patients with chronic conditions such as diabetes.
