Chevron unveils internal report on Calif. refinery fire

Chevron failed to pinpoint and replace a corroded pipe that contributed to a fire that broke out last August at its Richmond, Calif., refinery, according to a report from the company. Chevron also botched its response to the pipeline leak, the report stated. "The refinery's management and our entire work force are implementing a comprehensive set of actions, not simply to address the issues underlying the August incident, but also as part of our commitment to continuous improvement," Chevron said.
