NP creates tool to ID patients at high-risk for heart disease

Nurse practitioner Mariam Kashani created a clinical-decision support tool that identifies patients classified as low-risk for heart disease by the Framingham Risk Score but whose family history really puts them at high risk. The tool was used to identify patients for enrollment in a program to manage blood pressure and cholesterol and provide advice on heart-healthy lifestyle behaviors.
