Data breach affects 818 patients in Texas

Hope Hospice in Texas has announced a data breach affecting 818 patients that occurred when a staff member e-mailed patient reports through an unsecured channel in December and February. Compromised data included patient names, insurance data and clinical chart information. "The agency is performing a comprehensive review to further refine its policies and procedures related to patient privacy and security. Steps are also under way to further improve the security of the agency's operations," a company notification said.
