Doctors innovate with new practice models

Many doctors are revamping their medical practices to adapt to increasing demands from insurers and the government to bring down costs while improving care. Dr. Thomas Bellavia of New Jersey created a medical home that uses teams of doctors and nurses to offer more coordinated care for patients, while Dr. Mark Holthouse of California transformed his clinic into a subscription-based model that offers fitness coaching, acupuncture and other services.
