Physician group uses daily phone calls to reduce readmissions

Advocare, an independent physicians group, is using daily phone calls from care coordinators to patients as a way to reduce hospital readmissions. The practice has a financial incentive to do so in its contract with Horizon Blue Cross Blue Shield of New Jersey, which has found practices in its patient-centered medical home program are spending less than those that do not use the model.
