N.J. hospitals reduce readmissions 7.5%, data show

New Jersey hospitals have reduced readmission rates 7.5% since 2010, according to a report from Healthcare Quality Strategies. Medical director Andrew Miller said hospitals are working with nursing homes, hospices, physicians, county health agencies and pharmacists on ways to reduce preventable readmissions by better care management and patient follow-up.
