PCMHs are growing trend to improve quality, reduce costs

More health care providers are creating primary care patient-centered medical homes, using varied models, to improve quality, coordinate care and reduce costs. "We're seeing a coming together of our ability to manage populations in a delivery system that has enabled primary care to look at communities in a different way," said Jonathan Harvey, M.D., chief medical officer of Martin's Point HealthCare in Portland, Maine.
