Group wants personalized care plans to replace Liverpool Pathway

Britain's NHS Halton Clinical Commissioning Group said it supports recommendations to supplant the Liverpool Care Pathway with personalized end-of-life care plans for seriously ill patients. CCG officials said the group uses a care facilitator who works with physicians, nurses and other care team members in discussions of end-of-life care issues.
