Contractor work led to fatal blast at Black Elk Gulf platform, report finds

A Black Elk Energy-commissioned report blames contractors for a fatal blast at the company's Gulf of Mexico West Delta 32 platform in November 2012. Workers with DNR Offshore and Crewing Services failed to clear and isolate flammable vapors before welding a flange on an open pipe at the site, which caused the explosion, the study found. Such actions violated Black Elk's safety standards, the company said.
