Calif. to offer grants to improve self-management of chronic diseases

The California HealthCare Foundation intends to offer up to $2.37 million in grants over three years to providers in the state who can come up with ways -- using IT or the Internet -- to help patients with chronic diseases become better at managing their conditions. Almost half of all Americans have at least one chronic condition.
