Better discharge plan helps hospital reduce readmissions

Charleston Area Medical Center in W.Va., reduced readmission rates for heart failure and pneumonia patients by making its discharge instructions easier to understand with simple guidelines and reminders. The hospital also added telephone surveys and follow-up phone calls that help patients deal with symptoms or problems that could result in a readmission.
