Smaller health insurers to populate ACA exchanges

Some of the U.S.'s largest health insurance companies, including Cigna, Aetna and UnitedHealth, are avoiding Affordable Care Act exchanges in all but a few states. That has opened the door for smaller players, some of which will be offering policies on the commercial market for the first time.
