Project aims to help NYC facilities meet medical home, MU standards

A dozen community health centers in New York City, specifically in the Bronx, Queens, Brooklyn and Manhattan, are participating in a program designed to help them achieve meaningful use of health IT and meet standards for the medical home model. The project demonstrates a move "toward a quality-based system that rewards providers for focusing on prevention, coordinated care and better patient outcomes," the state's health commissioner said.
