Hospitals broaden view of community health, preventive care

Health care reform and new payment structures based on readmissions and quality are forcing hospitals to broaden their approach to community health and specific patient populations, including the chronically ill. Parkview Health in Fort Wayne, Ind., has been recognized for its work to improve community health. Nurses focus on prevention and help people manage chronic conditions. Registered nurse Connie Kerrigan, Parkview's manager of community nursing and women's health, says the goal is to understand what is happening in the community and "see how the gaps we're witnessing could be filled."
