Hospital opens Stay Healthy Clinic to reduce readmissions

Barnes-Jewish Hospital in St. Louis has opened the Stay Healthy Clinic, where hospital patients at risk for returning are seen within a week of discharge as a way to reduce high readmission rates. The follow-up exam is used to ensure patients understand their discharge instructions, can ask questions and are directed to a family physician.
