Nurse navigators tackle readmission rates at Ill. hospital

A team of nurse navigators at St. John's Hospital in Springfield, Ill., is part of a pilot project aimed at reducing hospital readmissions. The nurses work with physician offices to provide extra care for patients with chronic conditions. The hospital faces a penalty from CMS for having too many readmissions for heart failure, heart attack or pneumonia.
