Group issues transition guidelines for young diabetes patients

Clinicians who care for teenage patients with diabetes should give them and their future health providers a written summary that includes information on medications, past glycemic control, self-care assessments, and comorbidities to help ease transition from pediatric care to adult care, according to a position statement by the American Diabetes Association. The statement also recommends individualized care that enables patients to self-monitor glucose levels and arrange appointments to prepare for the transition.
