Some of the worst strategic mistakes in recent memory -- like Microsoft's decision to ignore the Internet, or Google's failure to notice the rise of social networking -- came about because smart people were too wedded to existing orthodoxies, writes Margaret Heffernan. Those blind spots stopped bosses from noticing big changes in their industries, and stifled their companies' ability to respond nimbly and creatively.
