States offer funding to encourage patient-centered medical homes

A report in the journal Health Affairs showed 25 states have adopted payment mechanisms in the wake of Medicaid and Children's Health Insurance Program changes to encourage the growth of patient-centered medical homes. The report found that 14 states allow performance-based provider payments, four have a shared savings model and eight focus PCMHs on team-based care.
