Ill. nurses make home visits to help discharged patients

Nurse care managers from Blessing Hospital in Quincy, Ill., make home visits to discharged patients to help them manage their health care and medications to ensure they do not have to return to the hospital. Administrative coordinator Julie Shepard said the program provides a bridge between discharge and the next visit to a doctor's office.
