Nurse-led programs help N.J. hospitals cut readmissions

New Jersey hospitals use post-discharge programs, many led by nurses, to prevent readmissions for heart failure patients. St. Joseph's Regional Medical Center has a clinic for heart failure patients and is planning a second for stroke patients. Chief Nursing Officer Maria Brennan says the goal is "to try to manage the patients at home" and ensure they get follow-up care.
