Health clinics in Va. report progress on PCMH efforts

The seven health centers and clinics that received grants in 2009 from the Richmond Memorial Health Foundation in Virginia recently announced progress in their efforts to adopt a patient-centered medical home model. The facilities, which used a website to exchange ideas and request feedback as part of the initiative, reported delivering care to about 28,000 patients.
