Philadelphia hospitals leverage health IT to lower readmissions

A report from the Health Care Improvement Foundation found that 18 hospitals in Philadelphia reduced readmission rates by 7% and saved $4 million in the third quarter. The hospitals demonstrated improved strategies for curbing readmissions and boosting care transitions, in part, because of their use of EHRs and other health IT systems, the report says.
