Ohio hospitals are improving follow-up patient care to reduce readmissions before October 2012, when Medicare will start docking payments if readmission rates are too high. The Ohio Hospital Association expects hospitals in the state to lose $17.8 million in the first year of the new payment plan. Health systems are looking at strategies to help patients comply with medication regimens, see their physicians as scheduled and get the help they need at home.
