St. Bernard Hospital in one of Chicago's poorest neighborhoods could be penalized by Medicare for high readmission rates even though officials say some return stays are tied to community conditions, such as poverty, that are beyond their control. Ingalls Memorial Hospital, also at risk of penalties under the new policy, is part of a coalition of hospitals working to reduce readmissions by helping patients transition from hospital to home and providing better care coordination.
