Baycare Health System's telemedicine program in Florida helps nurses track vital signs and symptoms of patients with congestive heart failure, hypertension and chronic obstructive pulmonary disease. Although some studies on telehealth programs have shown mixed results, Baycare's research found that in the program's first year, hospital readmission for heart failure dropped from 16% to 4%, and there have not been any readmissions in the program over the past year.
