At least 4,000 surgical errors known as "never events" -- things that should never happen -- occur in the U.S. annually, Johns Hopkins University researchers reported in the journal Surgery. Foreign objects are left inside patients about 39 times a week, while the wrong procedure is conducted or the wrong body part is operated on about 20 times per week, researchers estimated.
