The Tennessee Valley Authority's quality-assurance program for the safety of commercial parts used for the utility's Watts Bar 2 reactor in Tennessee experienced a "breakdown," according to a report from the Nuclear Regulatory Commission. The issue could prompt the NRC to issue fines and assume additional oversight of the project, the agency said. TVA is now assessing all components used at the project, spokesman Duncan Mansfield said, adding that the issues won't affect the unit's December 2015 startup schedule.
