Patient-centered medical home initiatives were operating in 26 state Medicaid programs at the end of 2012, providing a model for public and private payers, registered nurse Mary Takach of the National Academy for State Health Policy reported in Health Affairs. States began using PCMHs in programs for mothers and children but now have adopted them for patients with chronic medical conditions as a way to lower costs and improve care quality, she said.
