Most hospitals are focusing on reducing readmissions for patients with heart attack, pneumonia and heart failure because they trigger Medicare penalties, but a few have broadened the effort to include all causes of patient rehospitalization. Medicare next year is scheduled to add chronic obstructive pulmonary disease, coronary stent procedures, vascular surgery and coronary bypass procedures to its list of preventable readmissions that can lead to penalties.
