Patient-centered medical home development is a key strategy for improving primary care, leading to better outcomes and lower costs, according to Melinda Abrams, vice president of the Commonwealth Fund and director of its Patient-Centered Coordinated Care program. Abrams said at an Alliance for Health Reform briefing that "accessible, coordinated, team-based, proactive medical care" is needed for other delivery and payment reform efforts to succeed.
