The Patient Protection and Affordable Care Act has unleashed unexpected consequences in the insurance market for employees, retirees and their families as businesses reconfigure their insurance coverage offerings. Companies have reportedly been limiting coverage for spouses and retirees or discontinuing employer-provided coverage altogether, sending employees to the health-care exchanges created by the law. Experts say these results were largely unintended and stem from companies' efforts to control costs.
